Berry Street Youth Transitional Support Service

Youth Housing Case Management, Crises Accommodation, Counselling for those at risk of homelessness.

Merri Outreach Support Services (MOSS)

The Crisis Response Program is a short-term program that offers team case management to support people experiencing homelessness and/or housing crisis. For those consumers requiring longer-term case management they will seek to refer to the appropriate case management program, often a transitional support program. Housing Support for the Aged Program provides services to residents of Moreland, Darebin, Whittlesea and Nillumbik.

Brotherhood of St Laurence - Aged Care Services

The Brotherhood of St Laurence provides a range of Aged Care Services including case management to oversee home care packages.

InTouch

Provides services, programs and responses to issues of family violence in migrant and refugee communities. Interpreters on-site. InTouch deliver specialised and intensive case management services to refugees and Special Humanitarian Program entrants which includes Therapeutic group work Women’s support groups, Individual counselling, Arabic and Vietnamese Men’s Behaviour Change Program.

Banyule Community Health

Provides Complex Care Managers aim to help people access services they need to stay safe, independent and healthy as possible. This includes AOD case management, and mental health professionals.

Asylum Seeker Resource Centre

Foodbank and community lunch 5 days per week. Primary Health Care Services. Material Aid: including Myki credit, international calling cards, mobile phones and gift cards to buy household items urgently needed. General Access Program (GAP), and for a range of issues, including homelessness, family violence and social isolation. Case Management, Housing and rental support to those seeking asylum or with refugee status.
